In this special final episode, we focus on the role of personalised care in keeping people healthy within their homes and local community. 

We explore this topic through the lens of NHS @home, a national programme aiming to ensure people have faster access to more appropriate and targeted care within the local community and at home without necessarily having to attend emergency care or arrange GP  appointments.
