What can healthcare professionals do when individuals facing greatest deprivation are not only experiencing higher risk of exposure to the virus whilst already living with existing poor health and multiple comorbidities?

Through a series of practical examples and case studies showcased by our guests, this episode explores the role of personalised care in reducing health inequalities through shared decision making, multidisciplinary collaboration, local community connections, patient engagement and patient empowerment.

The role of Personalised Care in tackling health inequalities

Over a year-and-a-half has passed since Covid 19 is thought to have arrived in the UK and it’s believed that nearly a million people are still suffering from the effects of Long Covid, a condition also known as Post Covid Syndrome. And, with such a variety of symptoms in such a new illness, treating the condition is far from straightforward.

So how could a personalised approach to care help people suffering with the condition?

Nuffield Health has created a rehabilitation programme encompassing patient involvement, participation and co-design.
