Traditionally, anaesthetists have ventilated patients undergoing prolonged surgery with supra physiological tidal volumes to prevent collapse and improve oxygenation. But is this safe? Evidence from the critical care sphere suggests ventilator induced lung injury may be minimised with a more conservative approach.
