The inquest into Ashwarya Aswath’s death has heard heartbreaking and traumatic accounts of her final hours, as medical staff have broken down on the stand as they were asked about how her gravely ill condition was missed by so many.

Her death brought to light the dire staffing crisis in WA’s health system, and that was echoed by staff giving evidence.
