Last September, two-year-old Joe Massa was carried into Northern Beaches Hospital's emergency department at 7am - by 11am, he was gone.

Today we investigate how a healthy toddler died from what should have been a treatable condition, and why multiple systemic failures at every level of the hospital led to this devastating outcome.
