Healthcare and education are siloed from one another, but collaboration between these systems is essential to addressing the complex needs of the most vulnerable children and families. We have designed a comprehensive school model that partners with a family’s medical home to incorporate medical home staff into multi-disciplinary school teams, providing preventative care, developmental screenings, and care coordination while supporting the wellbeing of the whole family. We are focused on finding scalable partnership solutions that positively impact the health and wellbeing of communities.
