The Health Ombudsman has concluded that critical safety failures were a direct contributor to the death of Lerato Mohlamme, a psychiatric patient at George Mukhari Academic Hospital. The findings, released this Monday, emerged from a comprehensive investigation into patient safety incidents across Gauteng. The report details a tragic 2024 incident where a fire broke out in a hospital ward; while 17 other patients were successfully evacuated, Mohlamme did not survive.
