Both the Madlanga Commission of Inquiry and the Parliamentary Ad Hoc Committee were established in July 2025 as an emergency response to explosive allegations that South Africa’s criminal justice system has been "captured" by organized crime. The catalyst for both was a public media briefing on July 6, 2025, by the KZN Provincial Police Commissioner, Lieutenant General Nhlanhla Mkhwanazi, who claimed that a sophisticated drug cartel had successfully infiltrated the SAPS, the Judiciary, and the National Prosecuting Authority (NPA).
