The referral is made from a GP or other healthcare professional for individuals with health conditions or for those at risk of developing them. The aim is to increase physical activity and improve overall well-being.

Typically for adults the programme includes a 12 or 24 week structure exercise plan with a qualified instructor to meet individual needs.
