The coroner heard criticism of the shopping centre's security operations, including an unmanned control room during the attack and delayed alarm responses. Joel Couchy's former psychiatrist Dr Andrea Boris-Levac also faced scrutiny over deficiencies in his discharge from psychiatric care in 2020.
