National Children's Commissioner Anne Hollonds has lashed out at the ineffectiveness of Indigenous and children's care reform after a 10-year-old took his own life while in state care.

Ms Hollonds told Gary Adshead on 6PR Mornings there have been "decades" of Royal Commissions and enquiries every time a similar tragedy happens.
